Amentia (from Latin amentia — madness) is an acute exogenous psychosis characterized by clouding of consciousness with complete disorientation in one’s surroundings, incoherence of thought and speech, motor restlessness, and pronounced affective disturbances. It belongs to the group of disorders of consciousness and is one of the more severe forms of psychiatric pathology.
Clinical Picture
Amentia is characterized by a profound clouding of consciousness in which perception of the surrounding world and of one’s own personality is disrupted. Its principal symptoms include:
- Complete disorientation in place, time, and one’s own identity
- Incoherence of thought and speech
- Absence of goal-directed action
- Motor restlessness, often in the form of primitive, stereotyped movements
- Emotional lability (rapid mood swings)
- Fragmentary delusional ideas and hallucinations
- Disruption of the sleep-wake cycle
A hallmark of amentia is speech confusion — patients utter disconnected words or phrases with no logical link between them. Their speech is often incomprehensible to those around them and may be accompanied by inarticulate sounds.
Etiology
Amentia develops as a reaction of the central nervous system to severe somatic illness and intoxication. Its principal causes include:
- Infectious diseases with high fever (typhus, pneumonia, meningitis, sepsis)
- Severe intoxication (alcohol, drugs, medication)
- Massive blood loss
- Severe metabolic disturbances (liver and kidney failure)
- Postoperative complications
- Severe traumatic brain injury
- Debilitating somatic illness
Cerebral hypoxia, disrupted neuronal metabolism, and electrolyte imbalance play a significant role in the development of amentia.
Pathogenesis
The pathogenesis of amentia is based on diffuse damage to the cerebral cortex, predominantly affecting the functions of the frontal and temporal lobes. The development of the amentia syndrome is linked to:
- Disruption of oxidative-reductive processes in neurons
- Electrolyte imbalance
- Cerebral edema
- Toxic effects on nerve cells
- Disruption of cerebral blood flow
In amentia, a significant decline in the functional activity of the cerebral cortex is observed, confirmed by electroencephalography.
Diagnosis
Diagnosis of amentia is based on the clinical picture, medical history, and additional test data. Important diagnostic criteria include:
- A history of severe somatic illness or intoxication
- Characteristic clinical symptoms (disorientation, incoherent thought and speech)
- Pronounced affective and psychomotor disturbances
- EEG changes (diffuse slowing of the background rhythm)
- Laboratory findings indicating intoxication or infection
Differential diagnosis is made against other forms of clouded consciousness — delirium, oneiroid states, twilight states of consciousness, and other psychotic disorders.
Treatment
Treatment of amentia is comprehensive and includes the following directions:
- Etiotropic therapy — treating the underlying condition that caused the amentia syndrome (antibiotics for infections, detoxification therapy for poisoning)
- Detoxification therapy — infusion therapy with forced diuresis, plasmapheresis
- Correction of metabolic disturbances — normalizing water-electrolyte and acid-base balance
- Cerebroprotective therapy — drugs that improve cerebral blood flow and metabolism
- Symptomatic therapy — antipsychotics and tranquilizers to control psychomotor agitation
- Supportive care — adequate nutrition, patient care
Patients with amentia require constant observation and care, often necessitating hospitalization in an intensive care unit.
Prognosis
The prognosis for amentia depends on the severity of the underlying illness and the timeliness and adequacy of treatment. In most cases, successful treatment of the underlying condition leads to a gradual restoration of mental function.
Possible outcomes of amentia:
- Full recovery — seen in most cases with timely treatment
- Partial recovery — with residual cognitive deficit
- Transition to a chronic state — development of organic brain damage
- Death — in extremely severe somatic illness without adequate therapy
After amentia, patients often experience asthenic syndrome, which can persist for several months.
Historical Background
The term «amentia» was introduced into psychiatric practice by the German psychiatrist Theodor Meynert in the late 19th century. In his work he described the clinical picture of the condition and distinguished it from other mental disorders.
In the early 20th century, another German psychiatrist, Karl Bonhoeffer, studied in detail the connection between amentia and somatic illness and intoxication, contributing to a deeper understanding of the disorder’s nature.
In the current ICD-10 psychiatric classification, amentia is not listed as a standalone diagnosis but is considered within the category of organic mental disorders (F05 — Delirium not induced by alcohol or other psychoactive substances).
